INTRODUCTION

1. This form is to accompany any Ordinary, Lifetime or Associate Membership application whereby the applicant is aged 16 or 17 years.

2. The minimum age for any of these membership types is 16 years.

3. For the purposes of this form, "guardian" is defined as a person with legal custody of a child and having responsibility for the child’s support, health, and education, according to the Malaysian Guardianship of Infant Act 1961 (GIA).

4. For the purposes of this form, "child" is defined as a person under 18 years of age, according to the Malaysian Child Act 2001.
