"Harbingers of Lust" Spring 2024 Dance Series Intake Form

This form is to complete your registration for Chai's upcoming class series "Harbingers of Lust" from March 22 2024 - May 19 2024, performing at a SINnergy Productions "MUSICAL MAYHEM" Sunday May 19 2024 at the Victoria Event Center, AS WELL a music video style shoot of the dance! Music video date will be voted on in the FB group.

*PLEASE NOTE: There will be a two week break in this session, as I recover from a surgery.

Classes will be held as follows:

March 22nd 6-7PM

March 29th 6-7pm

April 19 6-7pm

April 26 6-7pm

May 3 6-7pm

May 10 6-7:30pm

May 17 6-7:30pm

Session is $250 if signed up before March 17th at midnight

Session is $275 if signed up after March 17th at midnight

Do you understand that this class series is working towards a dance performance at a production by SINnergy Productions, "MUSICAL MAYHEM" Sunday May 19 at the Victoria Event Center. Performing is not mandatory but highly encouraged. Are you committed to performing and making sure you are free of other commitments the evening of the show? *

There are two options of songs we can perform at this show! Please choose whichever one you'd prefer. Chai will tally the votes on the first day of class and the majority will be the once we perform *

Paris Is The Key To Your Heart - Anastasia Musical. / Think flapper dresses, fur shawls, very glamourous and romantic

Reefer Madness from the movie musical by the same name. / Think poodle skirts, fake joints, silly choreography, etc.

You will be added to a Facebook group called "Harbingers of Lust" where you'll be able to connect with your fellow dancers, costume options will be discussed and where videos of rehersals will be uploaded for review. Please add "Tala Small Wolf" (that's Chai's muggle name) as a friend on Facebook and she'll add you to the group. Please list your FB name in the Other option below *

Do you have any allergies/sensitivities I should be aware of? *

Please acknowledge the following statements: *

SINnergy Productions and Chai Tease is not liable for any injuries, illness, destruction of property, lost or missing items while services are provided. This includes at the dance studio, at photoshoot location and event venue.

I am over the age of 19 and can be around alcohol in a venue without issue

The group may be asked to perform again at another venue. This will be discussed within the group if such an occasion occurs.

The $100 deposit is non-refundable and non-transferable.

If you drop out of the routine after the third class, you will be subjected to a $50 rechoreography fee. This fee must be paid to participate in any further SINnergy Productions.

Payment of remaining tuition is due by March 22, 2024

I'm expected to attend every class date as listed in the FB group. If something comes up I will alert Chai at my earliest convinence. Missed classes will not be made up unless it's Chai Tease who cancels.

I am required to sign a waiver upon my first class at Passion & Performance Dance Studio. This is mandatory for all occupents on the premise.

SINnergy Productions is a safe space. We do not allow any homophobic, transphobic, racist, ageist, sexist, anti-sexwork, fatphobic comments our actions in our company. Breaking our safe space will lead to expulsion from the dance and company without refund.

I will do my best to try and make the date of the music video shoot as discussed in the Facebook group. Missing/skipping/or not partaking in the music video does not equate to a reduction in tuition fees.
