What are the advantages of TURP procedure?

After undergoing surgery, patients will notice a remarkable improvement in their overall quality of life vis-à-vis on medicines. The prostate surgery in Delhi performed at our hospital helps in releasing all the bothersome urinary symptoms due to an enlarged prostate such: -

  • Frequency of micturition.
  • Straining to pass urine.
  • Poor flow of urine and prolonged voiding.
  • Sense of incomplete emptying after passage of urine.
  • Nocturia i.e. waking up again and again in the night to pass urine.
  • Hesitancy i.e. the delay in initiation of passing urine.
  • Intermittency i.e., start-stopping of urination.
  • Urgency i.e. sudden strong desire to pass urine which may also be associated with leakage of some amount of urine.
What are the preparations to undergo TURP?
  • An ultrasound is the first investigation to assess the size of the prostate, amount of post void residual urine and the status of kidneys.
  • Serum PSA test is done to rule out prostate cancer.
  • Uroflowmetry is done to assess the pattern of urination whether obstructed or not.
  • A urine culture to document the absence of infection is mandatory before TURP surgery.
  • Complete blood tests, a chest x-ray and ECG are done to assess the fitness for surgery.
  • In case of diabetes and hypertension, an adequate control of both is ensured before planning the surgery.
  • In patients taking blood thinning medicines, they are advised to stop these medicines 5-7 days prior to the surgery.
How is TURP done?
  • TURP is traditionally done under spinal anesthesia. However, in case of apprehensive patients it can also be done under general anesthesia.
  • This prostate surgery in Delhi is performed in lithotomy position.
  • Using cystoscope, the urine passage through urethra, the prostate and bladder are visualized.
  • After this a thin tube called a resectoscope is introduced through the urethra up to the prostate and using a thin loop with a wire at the end that heats up with electric current, prostate chips are removed.
  • At the end of the procedure, a catheter is placed into the bladder which is connected to irrigation to wash the bladder.
What to expect in the post-operative period?
  • Patients stay in the hospital for one or two days post-surgery. During this time their bladder is constantly irrigated with saline.
  • Patients are given a soft diet during their stay.
  • Injectable antibiotics are given to prevent infections and pain killers are given for pain relief.
  • After adequate recovery patients are discharged on a catheter.
What to expect after discharge?
  • Patients are advised to take a proper bath every day.
  • Catheter care including how to empty the urine bag is explained to the patients and their attendants at the time of discharge.
  • Antibiotics and mild painkillers are given for 5-7 days post discharge.
  • Some amount of irritation due to catheter is expected and patients are counselled not to worry about it.
  • Some amount of blood in urine may be noticed for a few days especially after movement. This is normal and patients should not worry about it. Adequate hydration takes care of this problem.
  • Patients are also advised to avoid constipation and if need be, they are started on laxatives.
What is the follow up post surgery?
  • Patients are advised to follow up in hospital around 5-7 days post-surgery.
  • At this time the catheter is removed and patients are sent back home once they have voided satisfactorily post catheter removal.
  • Some amount of burning while passing urine can happen for a few days post-surgery which settles with time.
  • Patients may experience some urgency for one to two weeks.
  • Constipation must be avoided for 3-4 weeks post-surgery.
  • Patients are refrained from doing any heavy physical work/exercise for a month post-surgery.
  • An ultrasound with post void residual urine and uroflowmetry are repeated after one month to assess the outcome of surgery.
What are the alternatives to TURP?

TURP is the gold standard surgery for an enlarged prostate. However, in special circumstances prostate surgery can also be done endoscopically using lasers. The two common laser prostate surgeries in Delhi are:

  • Holmium Laser Enucleation of Prostate (HOLEP)
In this surgery, holmium laser is used to enucleate the prostate gland which is than pushed into the bladder and from there it is morcellated and taken out.
    Cost: starting Rs. 60000.
  • Green Laser Photo Vaporization of the Prostate
In this method, prostate chips are not removed; rather green laser is used to burn/vaporize the prostate tissue. This method is particularly useful for patients who have cardiac issues and in whom it is not advisable to stop blood thinners as this surgery is practically blood less.
    Cost: starting Rs. 175000
